I last saw 4.5 in an early development version, when it was still called 4.3. Since getting that additional 0.2 it has gained a lite version of Documents To Go (Word and PowerPoint only - Excel will come later in the year), allowing BlackBerry users to edit Office documents.
The rendering of HTML emails is promised, but I couldn't make it work with the current versions of BIS or BES (the prosumer and enterprise server platforms). The ability to search emails on the server is a plus, though, as you'll generally only keep the last 30 days or so on your phone.
The user interface has some gentle tweaks in places, with new fonts, new themes, and a revised email client. The web browser is greatly improved, and seems to have a much greater chance of rendering sites correctly. The device will even stream YouTube videos! The other big features are a voice recorder, and video recording on the Curve devices, both being a big deal in the BlackBerry world, but a bit of a yawn to anyone used to other platforms.
This update certainly isn't radical, but it's a worthwhile download if you can find a copy.
